Title: Very high HI - help!

vlm448 - January 4, 2008 07:56 PM (GMT)
I have been on a red-med machine. Leak rate averages .30 monthly
AHI 11.6
AI 4.5
HI 7.6
My sleep dr. recommends provigil for the high HI but I don't want to take daily stimulant. Still very tired and fatigued. Anyone encountered this and have suggestions? Thank you!!!!

Welcome to Apnea Board!

Unless you're dealing with very high pressures or other complications to your PAP therapy, it doesn't sound optimal to me.

The first thing I would look at would be leaks. An average leak of .3 litres is pretty substantial. I don't really understand the relationship between leaks and respiratory disturbances, but I can tell you that when my leak number goes up, so does my RDI.

When I used a full-face mask and endured significant, chronic leaks, I typically had an AHI between 5 and 7. Since I have been using a high-quality nasal interface, my leaks are usually zero and my AHI averages 2 or so.

What kind of mask are you using, and what is your pressure or pressure range?
